Privacy and Record Keeping

The information that is shared during therapeutic sessions with Amy Cox Counselling Services will be treated as confidential. This includes information provided verbally or in written form. All your personal information will be stored, along with session notes, in electronic form on a secure, encrypted storage device.

Confidentiality

There are circumstances where Amy Cox Counselling Services is legally obliged to share information about you and/or about our sessions. These situations are:

  • Where you have provided written permission to share information with another party.

  • If I consider you, or someone else, to be at risk of harm.

  • If I am made aware of a serious crime that has been committed.

  • If your counselling records are subpoenaed by a court of law.

Please note that Amy Cox Counselling Services is engaged in routine, professional supervision and will on occasion, discuss details of your case with her supervisor. Amy Cox Counselling Services will not disclose any information that will reveal your identity.

Contact Between Sessions

Email or telephone contact must be limited to practical arrangements such as bookings, rescheduling or cancelling appointment times.

Amy Cox Counselling Services are not available to support you outside of business hours. If you are faced with an emergency in between our sessions, please contact emergency services immediately on 000, Lifeline 13 11 14, Mental Health Line 1800 011 511 or the Kids Help Line 1800 551 800.

Termination of Counselling Contract

This counselling contract will end due to mutual agreement once therapeutic goals have been met, or once a pre-agreed number of sessions have been delivered. You have the right to terminate the counselling contract at any time with appropriate notice.

Minimum notice is 1 week and failure to provide this notice will result in an invoice being issued for one full session fee.

 

Cancellation Policy

If you need to cancel or reschedule an appointment, please let me know in advance. Cancellations made with less than 24-hour notice will be invoiced 100% of the hourly cost of session rate. Cancellations made with less than 72-hour notice will be invoiced 50% of the hourly cost of session rate.
